ONLINE PRIVACY NOTICE

  1. Introduction.
    • Web Site Owner. Sustainable Health, LLC (“Sustainable Health”) is the owner of this web site (“Sustainable Health Web Site”). Sustainable Health can be contacted by mail at 2897 Mapleton Avenue Suite 800, Boulder, Colorado 80301, by phone at 303-539-6525, or by e-mail at info@14erboulder.com. This online privacy notice discloses Sustainable Health information practices for this Sustainable Health Web Site, including what type of personal identifiable information is requested in order to make a purchase, how the information is used, and with whom the information is shared.
    • Web Site Visits. Sustainable Health collects certain information from Sustainable Health Web Site visitors. This includes, but is not limited to, the home server domain name, e-mail address, type of computer and web browser, what pages visitor accessed, and limited information about search requests. This information is used to solve technical problems and to calculate overall usage statistics.
    • Web Site Transactions. At times, Sustainable Health will need personal information regarding a customer or a prospect. For example, to process an order or provide a subscription, Sustainable Health may need to know a customer’s name, mailing address, e-mail address and credit card details. It is Sustainable Health’s intent to inform you before Sustainable Health collects personal information, such as user’s name and/or address on the Internet. If you tell us that you do not wish to have this information used as a basis for further contact with you, Sustainable Health will respect your wishes.

In order to access designated subscriber services and/or restricted areas within the Sustainable Health Web site, Sustainable Health will request a user to provide certain personal identifying information, which may include: name, postal address, e-mail address, telephone number, facsimile number, method of payment, and, if applicable, credit card number. Sustainable Health may request additional information necessary to establish and maintain customer’s account.
    • Service Quality Monitoring. Some Web site transactions may require a customer to telephone Sustainable Health, or Sustainable Health to call the customer. Sustainable Health will not contact you by telephone without your prior consent.
    • Information from Children. Sustainable Health does not collect or maintain information from users actually known to be under the age of 21, and no part of Sustainable Health’s Web sites are structured to attract anyone under the age of 21.
    • Lost or Stolen Information. Help keep your personal information accurate. If a customer’s personal information has changed, please e-mail the new information to Sustainable Health at the e-mail address shown in Paragraph 1(a). If a customer would like to review the personal information Sustainable Health has in its files regarding the customer, e-mail your request to Sustainable Health together with the description of a recent purchase.
    • Links to Other Web Sites. An Sustainable Health Web site may contain links to other web sites. Sustainable Health is not responsible for the privacy practices or the content of those other Web sites.
  3. Uses Made of the Information.
    • Limited Uses Identified. Without customer’s prior consent, Sustainable Health will not use your personal identifiable information for any purpose other than that for which it is submitted. Sustainable Health uses personal identifiable information to reply to inquiries, handle complaints, provide operational notices and in program record-keeping. Sustainable Health also processes billing and business requests related to Sustainable Health Web Site participation.
    • Marketing Uses. The information you provide to the Sustainable Health Web site may also be collected, used, analyzed and/or processed by Sustainable Health, or selected third parties on Sustainable Health’s behalf, for marketing and other business purposes. Before Sustainable Health uses the information, however, Sustainable Health will notify customers and offer customers the opportunity opt-out if a customer wishes not to have personal identifying information used in this way.
    • Stored Information Uses. Sustainable Health stores the information provided by customer. Stored information is used by Sustainable Health and Sustainable Health agents: to support customer interaction with the Sustainable Health Web site; to fulfill customer purchases; and/or to contact customer again about other Sustainable Health services and products.
    • Online Advertising. Some companies that help Sustainable Health deliver interactive on-line advertising, such as banner ads, may collect and use information about Sustainable Health’s customers to help Sustainable Health better understand the types of advertising or promotions that are most appealing to Sustainable Health’s customers. After it is collected the information is aggregated so it is not identifiable to a specific individual.
  4. Disclosure of the Information.
    • Mergers and Acquisitions. For the purpose of developing and expanding its business, Sustainable Health may share, rent, sell or buy business assets. In such transactions, customer information is generally one of the transferred business assets. Also, in the event Sustainable Health is acquired, customer information will be one of the transferred assets.
    • Agents. Sustainable Health employs or engages other companies and individuals to perform business functions on behalf of Sustainable Health. These persons are provided with personal identifying information required to perform their functions, but are prohibited by contract from using the information for other purposes. These persons engage in a variety of functions which include, but are not limited to, fulfilling orders, removing repetitive information from customer lists, analyzing data, providing marketing assistance, processing credit card payments and providing customer services.
    • Marketing Analysis by Third Parties. Sustainable Health reserves the right to disclose to third parties personal information about customers for marketing analysis; however, any information disclosed will be in the form of aggregate data that does not describe or identify an individual customer.
    • Disclosure to Governmental Authorities. Under certain circumstances, personal information may be subject to disclosure pursuant to a judicial or other government subpoenas, warrants or orders.
  5. Use of Computer Tracking Technologies.
    • No Tracking of Personal Information. Sustainable Health’s Web site(s) are not set up to track, collect or distribute personal information not entered by visitors. Our site logs do generate certain kinds of non-identifying site usage data, such as the number of hits and visits to our sites. This information is used for internal purposes by technical support staff to provide better services to the public and may also be provided to others, but, again, the statistics contain no personal information and cannot be used to gather such information.
    • Use of Cookies. A cookie is a small amount of data that is sent to customer’s browser from a Web server and is stored on the computer’s hard drive. Sustainable Health uses non-identifying cookies to provide easier site navigation. Sustainable Health’s Web site(s) can still be used if customer’s browser is set to reject cookies. Sustainable Health’s cookies do not generate personal data, do not read personal data from your machine and are never tied to anything that could be used to identify you.
    • Collection of Non-Identifiable Information. Sustainable Health may collect non-identifiable information from user visits to the Sustainable Health Web site(s) in order to provide better customer service. Examples of such collecting include: traffic analysis, such as tracking of the domains from which users visit, or tracking numbers of visitors; measuring visitor activity on Sustainable Health Web site(s); Web site and system administration; user analysis; and business decision making. Such information is sometimes known as “clickstream data.” Sustainable Health or its contractors may use this data to analyze trends and statistics.
    • Collection of Personal Information. Sustainable Health collects personal identifying information from customer during a transaction. Sustainable Health may extract some personally identifying information about that transaction in a non-identifiable format and combine it with other non-identifiable information, such as clickstream data. This information is used and analyzed only at an aggregate level (not at an individual level) to help Sustainable Health understand trends and patterns. This information is not reviewed at an individual level.
  6. Information Security.
    • Commitment to Online Security. Sustainable Health intends to protect customer personal information and to maintain its quality. To achieve information security and quality, Sustainable Health implements appropriate measures and processes, such as using encryption when transmitting certain sensitive information.
    • No Liability for Acts of Third Parties. Sustainable Health will exercise all reasonable efforts to safeguard the confidentiality of customer personal information. However, transmissions protected by industry standard security technology and implemented by human beings cannot be made absolutely secure. Consequently, Sustainable Health shall not be liable for unauthorized disclosure of personal information due to no fault of Sustainable Health including, but not limited to, errors in transmission and unauthorized acts of Sustainable Health staff.
  7. Privacy Policy Changes and Opt-Out Rights.
    • Changes to Privacy Policy. This privacy notice was last updated on December 7, 2020. Sustainable Health reserves the right to update its privacy policy statement at any time. A notice of any material change will be posted on the Sustainable Health Web site on this page for three (3) days prior to the implementation of such change.
    • Opt-Out Right. Customer and prospective customer has the right at any time to cease permitting personal information to be collected, used or disclosed by Sustainable Health and/or by any third parties with whom Sustainable Health has shared and/or transferred such personal information. Right of cancellation may be exercised by contacting Sustainable Health via e-mail at INFO@14ERBOULDER.COM, telephone or certified postal mail. After processing the cancellation, Sustainable Health will delete customer or prospective customer’s personal information from its database.
  8. Access Rights to Data.
    • Information Maintained by Sustainable Health. Upon customer’s request, Sustainable Health will provide a reasonable description of customer’s personally identifiable information that Sustainable Health maintains in its data bank.     • Corrections and Changes to Personal Information. Help Sustainable Health to keep customer personal information accurate. If customer’s personal information changes, or if customer notes an error upon review of customer information that Sustainable Health has on file, please promptly e-mail Sustainable Health at INFO@14ERBOULDER.COM and provide the new or correct information.
  9. Accountability.
    • Questions, Problems and Complaints. If you have a question about this policy statement, or a complaint about Sustainable Health compliance with this privacy policy, you may contact Sustainable Health by e-mail at INFO@14ERBOULDER.COM.     • Terms of Use. If customer chooses to subscribe to Sustainable Health’s services, customer’s action is hereby deemed acceptance of Sustainable Health practices described in this policy statement. Any dispute over privacy between customer and Sustainable Health is subject to the provisions of this notice and to Sustainable Health’s Terms and Conditions of Use Agreement which is hereby incorporated herein and which can be read here.
