Links to Other Web Sites. An Sustainable Health Web site may contain links to other web sites. Sustainable Health is not responsible for the privacy practices or the content of those other Web sites.
Uses Made of the Information.
Limited Uses Identified. Without customer’s prior consent, Sustainable Health will not use your personal identifiable information for any purpose other than that for which it is submitted. Sustainable Health uses personal identifiable information to reply to inquiries, handle complaints, provide operational notices and in program record-keeping. Sustainable Health also processes billing and business requests related to Sustainable Health Web Site participation.
Marketing Uses. The information you provide to the Sustainable Health Web site may also be collected, used, analyzed and/or processed by Sustainable Health, or selected third parties on Sustainable Health’s behalf, for marketing and other business purposes. Customer and prospective customer has the right at any time to cease permitting personal information to be collected, used or disclosed by Sustainable Health and/or by any third parties with whom Sustainable Health has shared and/or transferred such personal information. After processing the cancellation, Sustainable Health will delete customer or prospective customer’s personal information from its database.
